Market: Sentara Transplant Center has been around for nearly 40 years and is the only transplant center in Hampton Roads Region. We have performed more than 2,200 transplants. Nursing Excellence: We were the first hospital in the region to become a Magnet® recognized hospital. What you will do: Coordinate care with post-transplant patients. Facilitates clinic flow, transplant evaluation, and patient care management. Provide weekly education sessions to patients and support member. Arranges appointments with surgeons, social worker, financial coordinator, pharmacy and dietary.
