The work base for this position is Washington CHD. Provides services to Healthy Start prenatal and infant clients based on Healthy Start Standards and Guidelines (HSSG), Chipola Healthy Start Coalition (CHSC) System of Care and Well Family System (WFS) Dosages, inclusive of initial assessment, Healthy Start Care Coordination and referring clients to local agencies. Inputs prenatal and infant screens into the Department of Health, Health Management System (HMS). Maintains an individualized plan of care and Family Support Plan as necessary.
