Description The Veterans Case Manager is a key member of the clinical team, dedicated to supporting military veterans focusing on transition toward long-term recovery and integration into the community. This position serves the military veterans aiming for recovery, reintegration into the community, and independent living. Through addressing social determinants of health and removing structural obstacles, the Case Manager facilitates successful client outcomes. Key responsibilities include running therapeutic groups, coordinating internal and external client care, and utilizing a closed-loop referral system to ensure seamless service delivery.
