Job description

Job Details: Scheduled Hours: Monday - Friday, 7:30am-4pm or 8am-4:30pm RN: Delivers direct and indirect care to patients in an office setting utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation in coordination with other health team members. Effectively interacts with the patient, significant others, and other health team members while maintaining high standards of professionalism. Provides high quality care to a variety of patient populations. Demonstrates knowledge and skills necessary to provide high quality care and adherence to department policies and procedures/guidelines. What you will do: - Monitor and evaluate all aspects of patient care to ensure optimal health outcomes through multidisciplinary collaboration. - Maintain accurate, detailed reports and records. - Instruct and educate patients and caregivers on a variety of topics including but not limited to disease management, medication use, and lifestyle modifications. - Collaborate with care team members to efficiently manage tasks such as referrals, prior authorization, phone calls, results notification, and medication refills. - Prepare patients for, and assist with, e...
