Anticipated End Date: 2026-06-26 Position Title: Palliative Care Field Advance Practice Provider Job Description: Location: This field-based role enables associates to primarily operate in the field, traveling to client sites or designated locations as their role requires, with occasional office attendance for meetings or training. This approach ensures flexibility, responsiveness to client needs, and direct, hands-on engagement. This position will support the area of Wilson/Rocky Mountain and surrounding territory. The Palliative Care Field Advance Practice Provider is responsible for collaborating with company physicians, the patient’s other physicians and providers, and their family members to develop complex plans of care in accordance with the patient’s health status and overall goals and values.
