Department: 39110 Milwaukee South 11 - Home Health Aides Status: Full time Benefits Eligible: Yes Hou rs Per Week: 32 Schedule Details/Additional Information: Hours: Monday-Friday 8am-4:30 pm. Part time: 0.6 FTE to 0.8 FTE No weekends or holidays. Covering the following zip codes: 53146, 53151, 53227, 53214 and 53219. - You will drive to patients' homes in the above zip codes to bathe them. It is preferred that you live in one of the above zip codes or close to them- in order to have the best work life balance and drive time. - Reliable vehicle is also a must have. Pay Range: $20.80 - $31.20 Major Responsibilities: - Contributes to effective patient outcomes. Responsible to provide competent patient care assigned. - 1)Demonstrates competent home health aide skills. - 2)Promotes patients to reach functional status goals by providing assistance with ADL’s following the HHA assignment record in rendering patient care and maintaining safety. - 3)Notes changes in patient’s condition requiring further attention, informs the case manager, or the RN/Supervisor and documents accordingly. - 4)Understands the hospice philosophy, comfortable providing specialized care to the terminally ill. -...
