Overview The You First team is seeking a dedicated Lifestyle Program Specialist to support our heart health initiative. This role involves working directly with program members to enhance their health and reduce the risk of cardiovascular disease through personalized support and resources. Sample responsibilities: Member interaction is primarily conducted by phone and email. This position is funded by a cooperative agreement with the Centers for Disease Control and Prevention. This is a hybrid position based in Waterbury, VT. Strong attention to detail and a passion for improving the health and well-being of all Vermonters are necessary attributes for this position. The ideal candidate will have a background in nutrition, health coaching, motivational interviewing, or physical activity. - Review cardiovascular risk assessments (blood pressure, A1C, cholesterol, health history, and social determinants of health) - Utilize motivational interviewing techniques - Provide personalized health coaching to support lifestyle changes - Facilitate access to resources such as fresh produce (farmers markets/CSAs) and physical activity opportunities (walking programs, gym memberships, exercise c...
