Gemma Services has an IBHS/ABA program operating out of our Mt. Airy location. Gemma offers: The Care Coordinator is responsible for securing and sustaining linkages in the community to promote a positive outcome for youth and family. Responsibilities include engaging/re-engaging youth and families to identify additional needed resources for a successful discharge. Works with the family to identify appropriate referrals and supports the family with those referrals. Supporting the operations of the IBHS Program and ensuring required documentation is completed to meet all regulatory, contractual, and credentialing requirements; complying with operating policies and procedures established for the assigned function; communicating with appropriate personnel; responding to inquiries or requests for information; maintaining appropriate records and providing assigned reports.
