Department: 37280 Wake Forest Baptist Medical Center - LC: Patient Navigation Status: Full time Benefits Eligible: Yes Hou rs Per Week: 40 Schedule Details/Additional Information: Mon - Fri 8 a.m. - 5 p.m. Onsite Pay Range: $26.55 - $39.85 EDUCATION/EXPERIENCE: Bachelor's degree required. Bachelor's degree in Social Work, Sociology, Education, or Psychology preferred. LICENSURE, CERTIFICATION, and/or REGISTRATION: N/A: ESSENTIAL FUNCTIONS: Communicate effectively with patients, families and caregivers while demonstrating empathy, compassion, honesty, and cultural sensitivity. Identify and address barriers to care by connecting patient and their families with appropriate resources within the heath system and community. Collaborates with other healthcare professionals and the patient's care team to facilitate timely access to care and resources. Documents accurately and completely in EMR concerns patient encounters (i.e., barriers identified, education provided, resources utilized and any other further interventions and follow up).
