Job Summary This is a full-time professional position. This position is part of the Hospice interdisciplinary team and is responsible to develop and implement a care plan to provide spiritual care and support to the patient and family. The schedule for this position is Monday-Friday 8:30am-5:00pm; weekends as needed; 1 major or 2 minor holidays per year Responsibilities what you do: - Provide immediate and long-term assessment, and documentation, of the spiritual situation and needs of patients and families assigned to your care area. - Collaborate with the patient, the patient’s family, facility staff and the Hospice Interdisciplinary Group (IDG) to develop, spiritually & culturally sensitive, spiritual care plan components. - Provide and document spiritual care and counseling interventions to patients and families, to include the full range of sacraments and rituals of the patient and/or family’s spiritual/religious community. - Provide, and document provision of, appropriate and relevant end of life information and education to facility staff, patients and their families in compliance with current Centers for Medicare and Medicaid Services (CMS) Conditions of Participation (CoPs...
