The Care Coordination Nurse collaborates to organize individualized plans of care and communicates the plan to appropriate stakeholders. Practices professional nursing and promotes patient advocacy in accordance with the ANA Code of Ethics for Nurses. Maintains a current Ohio Nursing License.
