Posting Date 03/24/2026 578 Health Blvd, Daytona Beach, Florida, 32114, United States of America RN Case Manager - Position Highlights Location: Daytona - Hybrid - Remote (CKD) This is a hybrid/remote, field based role requiring a dedicated home office and the ability to travel to meet business and patients needs, including nephrology partner meetings. Though this position is primarily remote, it may require the RN Case Manager to be in the field 1-2 days per week in an assigned territory. Coverage includes Daytona, Deltona and surrounding areas. Territory assignments may change cased on business needs. Schedule : Monday - Friday: Flexibility is required to accommodate patient needs and nephrology partner availability. Employment Type: Full-Time: This position functions autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, cost-effective care while minimizing fragmentation of the healthcare delivery system for ESRD and CKD patients.
