The Lourie Center, Adventist HealthCare seeks to hire a Nurse Specialist who will embrace our mission to extend God’s care through the ministry of physical, mental and spiritual healing. As the Nurse Specialist you will: - Provides support services or consultation to families and staff regarding child’s diagnosis and health issues, including sleep, nutrition, safety, behavior, and other developmental and parenting concerns. - Provides community resources (healthcare, Medicaid, REM, SSI, DDA, respite) and other supports. - Educates families about MCITP services and obtain background information as needed for assessment report. - Participates in eligibility and family assessments and assist in the development of the Individualized Family Service Plan (IFSP). - Provides service coordination to families and collaborate with other professionals on multidisciplinary teams. - Attends IEP (Individualized Education Plan) meetings for medically complex children. - Facilitates information sharing and coordination of care between MCITP team and private med...
