Position Description: The Full Time Exempt, Level 1 Targeted Case Manager reports to our Director of Care Management.The Level I Targeted Case Manager provides targeted case management services within a Certified Community Behavioral Health Clinic (CCBHC). This role assists individuals in sustaining mental health and substance use recovery and gaining access to needed medical, behavioral health, social, educational, legal, and community services.
