Employment Type: Full time Shift: Day Shift Description: Position Purpose At Saint Francis Hospital , the Registered Nurse Case Manager coordinates patient care across the continuum, ensuring safe, high‑quality, and cost‑effective outcomes. This role partners with physicians, interdisciplinary teams, patients, and families to assess needs, develop and implement care plans, monitor progress, and facilitate timely transitions of care while advocating for the patient’s overall well‑being. What You Will Do: As a Registered Nurse (RN) Case Manager , you will serve as a key driver of a collaborative care management process that coordinates, monitors, and plans healthcare services throughout the patient’s illness and recovery. Responsibilities include: Coordinating care across the interdisciplinary team Assessing clinical, psychosocial, and discharge needs Monitoring patient progress and outcomes: Supporting safe, timely transitions of care Ensuring compliance with regulatory, accreditation, and utilization standards Required: Graduation from an accredited nursing school Preferred: Bachelor’s degree in Nursing 3 years of acute care RN experience required of diversified, progressive nursin...
