HealthPartners is looking for a Certified Adult/Geriatric or Family Nurse Practitioner to join our Community Based Palliative Care team. As a Palliative Care Nurse Practitioner/Care Coordinator, you will be part of the largest multi-specialty care system in the Twin Cities. This position will provide fieldwork with seeing patients in their homes and facilities as well as telehealth with virtual video and phone visits. Local travel and HIPAA compliant remote work area required.
