Description Advocate for disability inclusion while identifying and creating new opportunities for classes and community involvement. Build rapport with individuals, their support teams, and community members. Create and update all required 245D support documents and attend and facilitate meetings with support teams. Train staff on best practices for delivering person-centered services.
