Baystate Noble Hospital, Registered Nurse Case Manager Shift: 40 hours, 1st shift. Weekend rotation required. POSITION HIGHLIGHTS: Coordinate Patient-Centered Care: Facilitate collaboration among patients, families, and healthcare teams to develop and implement individualized care plans, ensuring high-quality outcomes and improved patient experiences. Manage Transitions and Discharge Planning: Conduct assessments, coordinate care resources, and oversee safe, efficient hospital transitions while maintaining continuity of care and supporting medical management initiatives. Ensure Compliance and Optimize Reimbursement: Evaluate admissions and ongoing stays using approved criteria, communicate with payers to maximize reimbursement, and adhere to all compliance regulations and hospital policies.
