Posting Date 04/17/2026 403 W Grand Pkwy SSuite T, Katy, Texas, 77494-8358, United States of America Dialysis Experience Required: Must have a Current CCHT: Local 6- chair DaVita SNF Dialysis dens are located at Paradigm at Katy 1480 Katy Flewellen Rd, Katy, TX 77494 & Avir at North Houston 303 Hollow Tree Ln, Houston, TX 77090. Teammates may be asked to support coverage at either facility. Dens operate Monday through Saturday. Teammates typically work 3 days per week with NO on call. (Saturday's typically rotate) Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a significant burden on patients and adding significant avoidable costs to the health care system. DSD Services teammates will have access to all the capabilities and resources DaVita has to offer, and will work to quickly leverage these to develop the infrastructure and rapidly grow the new service line.
