Assumes responsibility for the management of assigned coaching cases to ensure patients have ability to improve for the duration of their participation in the Wellness program. Using behavior change theory and motivational interviewing techniques, works together with the patient to identify and monitor progress on meaningful goals and lifestyle changes that will help patients develop healthier habits, leading to better health outcomes.
