Nurse Clinician (Nurse Navigator/Case Manager) - Emergency Department - 60% - ( 26002724 ) Description The University of Iowa Health Care, Care Coordination Division is seeking a Nurse Clinician to function as a clinical nurse expert within the Emergency Department and care coordinator for the assigned patient population facilitating seamless transitions across the continuum of care. The nurse functions as an administrative and clinical liaison for individuals to optimize quality health care delivery using clinical assessment/screening tools, education reinforcement, follow-up on the plan of care recommendations and guiding the utilization of cost-effective resources. This position will manage a specified population and work closely with other care coordinators during transitions of care as well as other interdisciplinary team members across the Continuum of Care. Duties to Include: Collaborate with nursing staff and other health care professionals in the development and review of standards and in planning, implementing and evaluating patient care for a specific patient population.
