Description SCHEDULE: Full-Time: Monday-Friday 8am-4:30pm, on call required. SUMMARY: Provide CTI services and carry a caseload. Develop person-centered care plans, share knowledge of community resources, provide linkage to resources, engage individuals in skill building, offer crisis management, collaborate with other providers, assist individuals in navigating complex systems, and work with informal supports. ESSENTIAL DUTIES AND RESPONSIBILITIES: - Complete all CTI, agency and state required trainings, as assigned. - Foster a beneficial working relationship with the partnering hospital by attending regular meetings with hospital staff and communicating effectively. - Provide care coordination services for a maximum of 15 individuals. Link individuals to supports in the community to promote community tenure and management of their behavioral and physical health needs. - Maintain documentation in the electronic health record according to the requirements of the CTI manual and agency policies and procedures.
