Job Summary: - Provides clinical expertise within the palliative care services team to improve quality of life and health for patients with complex chronic illnesses and approaching end-of-life illnesses. - Builds effective relationships with referring physicians to ensure excellent team follow-up regarding patient consults and to build an improved culture of collaboration among specialists, primary care physicians, nurses, and other staff to meet the needs of palliative care patients - Proactively identifies opportunities to improve the patient and family experience of care and to improve the efficiency and effectiveness of resources - Ensure compliance with legal, regulatory, and clinical policies and procedures Essential Duties and Responsibilities: conditions organization - Provide consultation services in palliative care - Provide education/guidance regarding the scope of Supportive Medicine/Palliative Care services to group physicians - Monitor and supervise Supportive Medicine home care delivery team(s) - Facilitate clarification of patient and family goals of care - Diagnosis, treatment, and management of acute and chronic health - Order, perform, and interpret laboratory a...
