Job Summary: - The Transfer Center Access Nurse serves as the primary liaison for transferring patients to a higher level of care facility either by request of the physician (HLOC Transfer) or patient/family (Lateral Transfer). This role involves coordinating the exchange of medical information, arranging transportation, and ensuring all required documentation is completed. The nurse will also provide support within the Transfer Center, including answering calls and facilitating patient placements. Essential Functions: - Serve as the primary contact for interfacility patient transfers. - Coordinate the exchange of patient medical information between facilities. - Arrange ground or air transportation for safe patient transfers. Complete all required documentation, including: - Memorandum of Transfer - Consent to Transfer - Reciprocal Agreement - PAN documents - Assist in the Transfer Center with phone management and patient placement. Educational Requirements: - Graduate of an accredited School of Nursing (Bachelor of Science in Nursing preferred). - Must be able to communicate effectively in English, both verbally and in writing. - Certification and Licensure - Current, unencumbere...
