Physicians provide a comprehensive range of primary and specialty care services at the 333-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus including at a network of Eskenazi Health Center sites located throughout Indianapolis. FLSA Status: Non-Exempt Job Role Summary The Community Health Worker will work with patients at the health center and community level to enhance understanding and adherence to treatment protocols, identify social determinants of health, and help patients to set and achieve their self-management goals. The CHW will document home social and environmental assessments timely, accurately and with good quality, identifying barriers to accessing appropriate healthcare or other concerns within the patient's home environment.
