Department: 34100 Milwaukee South 11 - Nursing Status: Full time Benefits Eligible: Yes Hou rs Per Week: 40 Schedule Details/Additional Information: Schedule: Monday - Friday 8am to 4:30 pm. (Manager is open to discussing FTE of 0.8-1.0) Additional hours required per job role: RN to sign up for three weekends per year back up on call. Two -night shifts back up/triage on call every 60 days. Holiday requirement: Two holidays per year. Service area includes the following zip codes: 53146, 53151, 53227, 53214, 53219 PREVIOUS HOME HEALTH EXPERIANCE PREFERED. Pay Range: $38.20 - $57.30 Major Responsibilities: - Assumes responsibility for caseloads consisting of patients in specialty areas. - Conducts initial assessment to obtain thorough physical, social and environmental data. - Establishes the plan of care in accordance with the physician orders and patient needs. - Provides direct patient care and adjusts nursing care processes as needed. Identifies problems, modifies nursing care plan, and evaluates outcomes within an appropriate time frame.
