Overview: This is a triage position. The RN will work with a group of physicians, fellows, and NPs to provide care coordination for patients. The RN will communicate with patients, families, and ancillary services through Epic telephone encounters/My Chart encounters and via phone. Care Coordination activities consist of addressing medical issues/concerns, communicating test results, medication refills/prior authorizations, patient/family education, and communicating with ancillary services such as dieticians, home care companies, insurance companies, pharmacies, as well as many others. - Full-time, 40 hrs/wk | Monday-Friday, dayshift - Minimum- $36.90 per hr - Pay to commensurate with experience - Looking for experienced RNs Job Description Summary: Assists patients to move rapidly, uneventfully, and as effectively as possible through their episode of care while maintaining quality and safety as a priority.
