Employment Type: Part time Shift: Rotating Shift Description: Mercy Medical Center a member of Trinity Health Of New England is looking for an experienced Behavioral Health Specialist to come join our team of highly specialized integrated behavioral health professionals. The Behavioral Health Specialist provides behavioral health consults, assessments, individual treatment, family consultations, care coordination, referrals, and participates in multidisciplinary teams primarily within the hospital and emergency room.. What will you do: . Minimum Qualifications - Conducts assessments in various settings including crisis assessments in the emergency department. - Builds caring relationships with target population to promote positive health and social service outcomes. - Improves access to health and human services for target population through in-person interactions, telephone contacts, and coordination of referrals. - collaborate with team members to determine care plans from admission to discharge, with family and service providers. - collaborate with external agencies or facilities to coordinate referrals to care and will direct any appropriate follow-up. - Education: Master’s deg...
