Employment Type: Full time Shift: Description: POSITION PURPOSE: Provide quality services in a community environment with the skill sets to manage complex situations related to individuals who have experienced homelessness, potential substance use/behavioral/emotional problems. WHAT YOU WILL DO: Client Assessment : Conduct initial assessments of individuals experiencing homelessness to identify their needs, barriers, and strengths. Case Planning : Develop individualized service plans in collaboration with clients, setting goals and outlining steps to achieve housing stability and self-sufficiency. Resource Coordination : Connect clients with community resources such as shelters, food assistance programs, healthcare services, mental health counseling, substance abuse treatment, and job training programs. Advocacy : Advocate on behalf of clients to access housing, public benefits, legal assistance, and other necessary services. Crisis Intervention : Provide immediate assistance and support during crises such as housing emergencies, mental health crises, or substance abuse relapses.
