Title: SOCIAL WORK - COUNSELOR I State Role Title: Social Workers, All Other Hiring Range: $58,735 - $81,054, Commensurate with experience. Pay Band: 3: Agency: Dept Behavioral Health/Develop: Location: Eastern State Hospital: Agency Website: https://dbhds.virginia.gov/facilities/esh/: Recruitment Type: General Public - G Job Duties: This position provides social work services and discharge planning for individuals at ESH. Develop and implement person-centered treatment and discharge plans in collaboration with seriously mentally ill consumers and other member of the interdisciplinary treatment team. Serve as link between community agencies and others that support consumer’s recovery and return to community. Minimum Qualifications: Minimum of a Bachelor’s in Social Work (BSW) is required. Experience providing case management and discharge planning services to the mentally ill population. Valid driver's license. Additional Considerations: Experience with community resources for mentally ill population.
