The Chase Center is an adult residential facility. In this role you will provide general case management duties including intake assessment, individual and group counseling, case record management, discharge preparation and aftercare services. You will uphold all Agency policies and procedures pertaining to the safety and security of clients, Agency property and the community. You will also be a role model to the clients while providing a safe environment, facilitating counseling, advocacy and support. The program administers assessments, monitors clients' compliance with service plan goals, and involves clients in services that address their criminogenic tendencies. Through case management, supervision, and provision of evidence-based groups, clients gain the tools and support necessary for successful community re-entry.
