JOB SUMMARY: Assesses, plans and implements specific physical therapy treatment programs for individual patients in Acute Care Rehab settings across the Legacy system, at multiple sites. Responsibilities: Evaluation and Treatment Implementation: Conduct comprehensive evaluations of patients across all age groups—from neonates to geriatrics—using age- and disability-appropriate assessments to establish baselines and develop individualized treatment plans that promote functional improvement.
