Job description

Qualifications: - Provide professional social work services and interventions, including psychotherapy, individual/ family/group therapy and interventions, psychosocial assessment, risk assessment and management, crisis intervention and discharge planning, to clients with severe and persistent mental illness. - Develop and implement creative treatment, recovery and therapy plans, and support services and programs responsive to clients’ needs following according to the best practice and consistent with the Ontario College of Social Workers and Social Service Workers (OCSWSSW). - Engage in client- and family-centered care. - Engage client, family and caregivers in clinical interventions and risk reduction strategies with the interdisciplinary team. - Advocate on behalf of clients, families and caregivers for appropriate services and resources. - Collaborate, consult and liaise with the interdisciplinary team and community service providers. - Provide timely and comprehensive written assessments, treatment and discharge reports. - Participate in the teaching, education and supervision of Social Work students. - Maintain appropriate documentation and data collection, in accordance with...
