Position Summary: The Care Manager Associate - Hybrid Position (3 days/week in office) Will provides essential support to the Care Management (CM) team by facilitating member engagement, coordinating services, and ensuring timely documentation and follow-up. This role is critical in maintaining continuity of care and supporting the administrative and operational needs of the care management process.
