Work Location: Remote with onsite as needed Work Hours: Full time: The Community Patient Navigator is a non-clinical, paraprofessional assistant to radiologists and clinical staff, serving as a liaison to patients and referring physician community. This role is dedicated to assuring patients receive a variety of result types as well as understand appropriate follow up measures for continuity of care. The responsibilities are wide-ranging, including communication with patient care team, identification of barriers to care, as well as detailed tracking of the patient journey.
