GENERAL DESCRIPTION: This is a grant-funded position focused on providing case management and rental assistance to chronically homeless individuals and families in Marion and Polk Counties. The Case Manager will provide direct services to chronically homeless individuals, which include; eligibility screening, program enrollment, participant follow-ups and check-ins. Case managers will also be responsible for processing required paperwork, data entry and other general program supports. This position will directly align with the Housing First initiatives. Schedule: Flexible to work 7:00 AM -3:30 PM or 11:00 AM -7:00 PM. Weekend availability required. MINIMUM QUALIFICATIONS The requirements listed below are representative of the knowledge, skill, and/or abilities required. EDUCATION and/or EXPERIENCE: A High School Diploma or GED and two years of related social service experience; or an equivalent combination of education and experience. Ideal candidates will possess a college degree or specialized training with two or more years of homeless service delivery experience.
