We are currently seeking a Full Time Case Manager for our St. Bridget's Residence Shelter , located in Newark, NJ . Job Duties: - Responsible in assisting clients with the appropriate service referrals as well as developing a service plan to enable clients to meet their potential goals. - Linking individual clients to services with program activities that suitable for clients needs. - Responsible for continuity of care and assuring clients are maintaining productivity.
