SIGN ON BONUS!!: Centene is hiring an LTSS Service Care Coordinator for our Illinois market, supporting our Medicaid Aging and Physically Disabled population. In this role, you will conduct in-home visits to complete assessments, coordinate care, and assist members with connecting to providers and DME services. This is a 75% field-based position covering the Hanover Park, IL area. Candidates must reside within 30 minutes of this area. Schedule: Monday through Friday, 8:00 a.m. – 4:30 p.m. Position Purpose: Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with developing personalized service care plans/service plans for long-term care members and educates members and their families/caregivers on services and benefits available to meet member needs.
