Rivers Health is seeking a full-time Physical Therapist to perform the duties below: JOB SUMMARY: Entails initial evaluation and re-assessment of patients referred to the department, planning and administrating treatments and documentation of patient’s status and progress. JOB RELATIONSHIPS: Responsible to : Director of Rehabilitation Nature of supervision received : Indirect Positions directly supervised : Physical Therapy Assistants, Rehab Techs, Students, and Volunteers. ESSENTIAL JOB FUNCTIONS: The following is a summary of the major job functions of this job. The employee may perform other duties, both major and minor, which are not mentioned below; and specific functions may change from time to time. - Maintain and upgrade evaluation and treatment skills. - Evaluate patient's problems subjectively and objectively and document data via SOAP format, set goals, plans, and implement treatment program. - Assess patient progress and alter treatment plan as needed. - Provide accurate and professional documentation as required by departmental policy.
