Carry out patient care tasks and medical orders as delegated by physicians, APPs or other health care personnel following the Care Model Process. POSITION RESPONSIBILITIES: - Maintains confidentiality of sensitive information. - Conducts pre-visit planning, visit process, and other Care Model Process workflows. - Performs accurate and timely charting to support patient care and service. - Obtain health history information.
