As a Social Worker with Vancouver Coastal Health, you will: - Work as part of an interdisciplinary team to provide outreach services to individuals in the community who have difficulty accessing services with serious substance use disorders and other complex medical and psych-social needs such as mental health and/or primary care needs. - Provide comprehensive Social Work assessments, connects clients with health care and community services, develops, implements and adjusts individualized treatment plans in collaboration with internal team, affiliated prescribers and other treatment team members and provides counselling according to a variety of models. - Participate in team case conferences/meetings to coordinate and manage client care issues, aids clients access to needed services, establish and maintain relationships, liaises with staff of emergency rooms, inpatient psychiatric uni...
