FULL TIME EMPLOYEE BENEFITS: PHC is seeking an experienced Physician who has adult and pediatric urgent care or emergency room experience as well as and understanding of the primary care medical home model to join our welcoming and diverse community of providers. We are considering fulltime, part-time or per diem schedules.JOB SUMMARY:This provider will deliver patient-centered and culturally appropriate care to a diverse patient population and specifically manage patients’ acute conditions in both in-person and tele-health modalities including any urgent social and behavioral health concerns This provider will work closely with a collaborative team to ensure a proper bridge to the patient’s primary care medical home and avoidance of improper emergency room utilization.Responsibilities...
