The Aboriginal Health Practitioner (AHP) is a vital member of Wunan Health’s’ multidisciplinary team (MDT), providing culturally safe, person-centred care for individuals and families. Key Responsibilities: Clinical Care - Support GP and practice operations by providing high-quality, patient-centred care within scope of practice and in line with best available evidence - Duties include triage, immunisations and injections, wound care, ECGs, pathology collection, health assessments, chronic disease monitoring and support, and assisting medical practitioners with procedures. This includes spirometry and basic audiometry, with training and competency support provided. - Deliver routine primary care services including preventative screening, patient monitoring, follow-up, and care for acute, chronic and complex conditions. - Communicate clearly with patients, providing explanation, reassurance, support and advocacy throughout care. - Maintain accurate, timely clinical documentation and ensure all clinical activity meets professional, practice and accreditation standards. - Work collaboratively with GPs, nurses, allied health and the broader multidisciplinary team to support coordinated...
