AOD Case Manager

Job description

The AOD Case manager is responsible for ensuring an effective, high-quality delivery of rehabilitation services to individual clients. This will involve building strong and influential relationships, for the purpose of providing alcohol and other drug (AOD) specialist support required to facilitate recovery and rehabilitation from (AOD) problematic use, including volatile substances and co-morbidity. Provided support is to be culturally sensitive and compliant with the structured program and service delivery agreements. Key Responsibilities: - Ensure client admission file is completed in accordance with regulations, including assessment and signed informed consent paperwork to access other relevant information. - Work with clients to develop a personal Recovery plan. - Monitor and record all client needs and progress, including daily appointments. - Provide or facilitate both individual/ group therapy and counselling sessions. - Coordinate client appointments, business and recreational activities / outings as required, including family visits and cultural activities. - Liaise with other stakeholders, agencies and service providers in a prompt and professional manner. - To develop a...
