The mission of the Coordinator Clinical Care is to identify high risk patients and help coordinate resources to facilitate care, improve health outcomes, and to reduce avoidable hospitalizations and re-admissions. Required qualifications: - Graduate of school for medical assistants and certificate. Or - Coursework/Training: Diploma/certificate from the military in Medical Specialist Course. - 2 years Directed patient care at MA level or greater.
