Job Summary: The Manager, PACE Operations has responsibility for all disciplines and services provided through the Prisma Health Senior Care center as part of the Program of All-inclusive Care for the Elderly, including all administrative responsibility related to direct operation of the center. Essential Functions: All team members are expected to be knowledgeable and compliant with Prisma Health's purpose: Inspire health. Serve with compassion. Be the difference. Coordinates the delivery of health care in the home and in the PACE center through the supervision of staff responsible for ensuring participant needs are met 24 hours per day, seven days per week. ​ Coordinates all center activities including care planning and implementation, preparation for interdisciplinary conference and daily meetings, family conferences and all related follow up. Directly and indirectly supervises center staff, including nurses, therapists, social workers, transportation, and administrative staff.
