Our Company Haven Hospice: Overview: The Dementia Program Activities Coordinator will work with dementia patients receiving hospice care at home and in facilities to find activities that engage the interest of persons with Alzheimer's disease and other dementias. The Coordinator models activities to family caregivers and facility staff. The Coordinator will also perform bathing consultations in which they will model strategies to maximize the comfort of both the person with dementia and their caregiver. Additionally, the Coordinator will function as an IDT member for the patient, providing information to other care team members during IDT and individually, as appropriate. Responsibilities: - Speaks with family members/caregiver, facility staff and Haven team to identify: - Areas of interest and engagement opportunities for patient - Barriers to bathing and/or other ADLs - Demonstrates advanced knowledge of providing engagement and care to patients with dementia, as well as available community resources for patients and caregivers - Demonstrates proficiency in active listening skills and exhibits a non-judgmental, empathetic professional persona - Schedules on-site visit with patien...
