What you’ll do: We’re looking for an experienced RD/CDCES or RN/CDCES – referred to as a Diabetes Care Specialist at Ryse – to support patients virtually and in-person at our current and future clinics . You’ll build deep relationships with our patients and support them on their journey to better health while working closely with a top-tier team. Day-to-day you will: - Support patients in better health – measured by blood sugar, blood pressure, and lipids – at lower cost - Develop and implement patient care plans across the domains of diet, exercise, sleep, and stress management - Collaborate with prescribing providers on medication choices - Empower patients to reach their health goals by using motivational interviewing techniques and continuous glucose monitor (CGM) data - Coordinate care with R...
