Social Worker - Hospice, Grief & Bereavement (12-Month Contract)

Job description

Key Responsibilities: - Provide comprehensive biopsychosocial assessment, develop collaborative goals and create effective care plans with clients with life limiting illness and their caregivers/families. - Complete risk assessments and crisis interventions within a psychosocial framework. - Support Advanced Care Planning conversations (ACP) and provide related education to clients, caregivers and families - Assess grief support needs from an evidence based model and tailor interventions (social, educational, clinical) appropriately - Provide individual grief counselling and facilitate bereavement groups - Maintain an understanding of relevant legislation such as Health Care Consent Act, Medical Assistance in Dying, PHIPA, Substitute Decisions Act and their application to practice. - Understand and apply current norms of practice and best practices in Hospice Palliative Care and Grief and Bereavement. - Perform other duties as assigned.
