Job title: Aboriginal Health Worker Job Summary: We are seeking a dedicated Aboriginal Health Worker to provide culturally safe Case Management, Transition of Care, and Friends and Family support to Aboriginal parents and/or carers participating in a non‑residential day rehabilitation program. This role prioritises families with children aged 0–5 years and extends support to families with children up to 16 years of age, for those involved in AOD services and child protection interventions. The successful applicant will work within our stepped‑care, transdisciplinary model to promote the health, wellbeing, and long‑term outcomes of clients and their families. As part of the Yadhabadiwa program, Aboriginal Health Workers will work flexibly across Transition of Care, Friends and Family support, and Case Management, contributing to coordinated, holistic care.
